The federal government has set a timeline for stricter oversight of health insurance for low-income adults in California. San Bernardino County officials announced that new rules regarding work and community engagement will apply to specific Medi-Cal members beginning in 2027.

Under House Bill 1, adults ages 19 through 64 who receive coverage through the Affordable Care Act expansion must meet new standards to keep their benefits. This group, referred to as the New Adult Group, will need to demonstrate participation in work, volunteering, or education. The requirement demands 80 hours of activity per month.

Accepted activities to satisfy the 80-hour threshold include paid employment, self-employment, or in-kind work. Members may also count volunteering, community service, or participation in qualifying job-training programs. Attending school at least half-time qualifies for the full requirement. Alternatively, a monthly income of at least 580 dollars, based on the federal minimum wage, satisfies the mandate. Beneficiaries can combine these activities to reach the total.

Eligibility reviews will shift from an annual cycle to every six months for those affected. A member who completes a renewal in March 2027 will face another review in September 2027. Those in other coverage groups will maintain their current schedules. Exemption from the work requirement does not automatically exempt a member from the six-month renewal process.

Certain individuals are excused from the work mandate. These exemptions include people who are pregnant or receiving postpartum care, and parents or caretakers of children age 13 or younger. Individuals with medical conditions or disabilities that prevent compliance are also excused. People with Medicare coverage, American Indian or Alaska Native status, and certain former foster youth under age 26 are not subject to the new hours requirement.

The California Department of Health Care Services began notifying members in February 2026 via text message. Mail outreach letters went out in August 2026 to current New Adult Group members. These letters are available in multiple languages. San Bernardino County is including this information in application materials for new and returning members through the end of the year.

County staff will review available information to determine if a member must meet the work requirements or qualifies for an exemption. If additional data is needed, the county will send instructions on what to provide and by when. Members can use the BenefitsCal online portal to manage benefits, report changes, upload documents, and review notices.

Officials advise members to keep mailing addresses, phone numbers, and email addresses current with the county. Members should check mail and online accounts regularly for notices. Renewal forms and requested information must be returned by stated deadlines. Changes in income or household circumstances must be reported as required.

San Bernardino County will continue working with community partners, DHCS, and the Centers for Medicare and Medicaid Services to share updates. Additional details on verification requirements and forms will be released as guidance becomes available.
